Athletes must have a current Sports Medical Form and Health History Update Questionnaire on file. Physicals are good for 365 days.
Health History Update Questionnaires are good for 60 days prior to the start of practice and are due each sports season.
If your athlete has ASTHMA and it is indicated on your phyiscal or Health History Questionnaire, you will need to have the doctor fill out the attached Asthma Action Plan. Along with this plan is the self-medication form that needs to be signed by the parent and athlete.
